The Indiana State Department of Health (ISDH) oversees the Immunization Information System known as CHIRP (Children and Hoosier’s Immunization Registry Program). CHIRP's primary function is the gathering, prediction, organization, and dissemination of vaccine information to certified healthcare providers throughout Indiana. These providers encompass private medical establishments, local health departments, hospitals, schools, and pharmacies. CHIRP operates in accordance with Indiana Code 16-38-5, granting providers and their authorized representatives access to its data. Moreover, individuals have the right to retrieve their vaccine records or those of their dependents from an authorized CHIRP provider or directly from the Indiana State Department of Health.
As a recent enhancement to the current system, ISDH has introduced the MyVaxIndiana Immunization Portal, offering individuals the ability to examine their vaccine records from any computer. This process initiates with a registered CHIRP provider generating a unique Patient ID number (PIN) for authorized access to the requested record. Parents or legal guardians may obtain a PIN for reviewing their dependents' immunization history, while individuals have the same privilege for accessing their own records. Users are presented with various viewing and saving options for their data. These alternatives include printing and storing the Official Immunization Record, downloading the record in a file format compatible with their Personal Health Record system (PHR), or opting to have it sent via fax or regular mail.
